Three Step Process
Begin adding doctors and their patients in your OB Connect account. Doctors will have access to their patients through their own online account. Patients will be connected via the ThriveBaby app. Utilize CPT Code 99453 per patient for one-time setup and education.
Start receiving generated patient health data in real-time through an easy-to-use dashboard. Utilize CPT Code (99457 & 99458) or 99091 for professional time dedicated to monitoring services per patient for every 30-day period.
OB Connect is a virtual care platform that provides you with convenient, engaging, and precise care, directly from your doctor’s office to the patient’s home. OB Connect partners directly with healthcare providers to bring the patient educational content, weekly resources, and much more through the ThriveBaby mobile app. Additionally, you can monitor your patient’s health by tracking their weight and other vital signs right from their home.
Telehealth, or some times referred to as Telemedicine, includes a variety of tools and platforms that allow clinicians to connect with one another as well as with patients. Take a look our 2020 Telehealth Essential Guide to learn everything you need to know about Telehealth.
To best implement Telehealth please follow the guidelines in the American Medical Association’s Telehealth Implementation Playbook.
Remote patient monitoring, or RPM, uses digital technologies to collect health data from individuals in one location, usually the home, and electronically transmits that information securely to health care providers in a different location for assessment and recommendations.
Virtual visits take place between patients and clinicians via communications technology — the video and audio connectivity that allows “virtual” meetings to occur in real time, from virtually any location. A virtual visit can be a videoconference between a doctor and a patient at home. It could mean that a patient can interact with an offsite medical specialist via a high-definition conference hookup at his or her local clinic, instead of traveling to another city.
Please follow the guidelines from the Agency for Healthcare Research and Quality on “How to Obtain Consent for Telehealth“.
OB Connect provides a unique and personalized approach to prenatal care. While on the program, the patient will access educational content hand-selected for them. Additionally, they may receive the latest in prenatal research, updates about your practice, information on new classes and educational opportunities, and anything your care team would like the patient to know – all at the patient’s fingertips via the ThriveBaby app!
Please read our interview with Dr. Kelly Leggett, an OB-GYN at Cone Health and their Chief Clinical Transformation Officer, about how they are using Telehealth to support their pregnant patients. She explains how they successfully implemented 5 total in-office visits (through term) for low-risk patients and using a hybrid model for supporting high-risk patients.
OB Connect is a program that partners with you from the patient’s first OB visit through their sixth-week postpartum visit.
